参加費特別割引（〜2月3日迄に申込むと参加費は$299）
が適用されます。

1. Letter of Sponsorship（推薦状）
2. Membership Fees（初回年会費→2010年度分）
ただし、JCCの会員証明書を添付する必要があります。証明書を希望される方は学会事務局 (edu@jcc.gr.jp、 または TEL: 03-5802-0112）までご連絡下さい。JCCの会員証明書は、PDF形式のファイルでお送りします。

- International Associate Membership 用の申込用紙
をダウンロードします
- CV Society Registration and Hotel Reservation Form（参加登録申請書：ダウンロードした申込用紙の1〜3ページ）に必要事項をご記入ください
- International Associate Membership Application（入会申込書：ダウンロードした申込用紙の5ページ）に必要事項をご記入ください
- 日本心臓病学会(JCC)事務局 (edu@jcc.gr.jp、TEL: 03-5802-0112）に［会員証明書］の発行を請求してください
- Areana Quinoes (aquinone@acc.org)に、参加登録申請書(3枚）、入会申込書（1枚）、会員証明書（事務局が発行）をメールでお送り下さい。
※ 3ページ目の「11. Registration Category」は、□ACC International Associateにｘを入れて下さい。
※ 3ページ目の「CV Society Affiliation」には、"Japanese College of Cardiology" とご記入下さい。
※ 5ページ目の記入方法は、こちらをご確認下さい。
